Common questions
Is this for a shotgun certificate or a firearms licence?
Both. The same medical report is used for shotgun certificate and firearms certificate applications, new grants and renewals.
What is the GP proforma?
It's the medical form your police force asks a doctor to complete when you apply for or renew a shotgun or firearms certificate. If your own GP doesn't offer it, a GMC-registered doctor completes it for you using the records we request from your GP.
How does it work without an appointment?
You fill in the short form and pay £69 online. We request your medical records from your GP. A GMC-registered doctor reviews them and completes your medical report. There's nothing to attend.
What do you need from me?
Your name, email, phone number and address, your GP's name and address, and your signed consent so your GP can release your records to us.
Can I use this for a renewal?
Yes. A shotgun or firearms renewal uses the same medical report as a new application.
What is in the medical report?
A GMC-registered doctor's report based on your GP records, in the form your police force's firearms licensing team asks for. Call 0808 188 9112 if you want to check anything about how your force wants it.
How much does it cost?
£69, fixed. You pay online when you apply and there's no appointment to book.
What if my GP is slow to send my records?
We request them for you. If you'd like an update on where things are, call 0808 188 9112.
Can I apply by phone?
Call 0808 188 9112 and we'll talk you through it. The application and payment are done online.
Can I change or cancel my application?
Call us on 0808 188 9112 and we'll help.
