The DVLA D4 medical with a GMC-registered doctor, who completes every section of your D4 form at the van. A free retest is included if you need one. It's £49.99 online or £54.99 at the van.
Do I need to bring the D4 form?
No. If DVLA has sent you a D4 form, bring it and the doctor will complete it. If not, we have spares at the van.
What should I bring?
Your photo driving licence, your glasses or contact lenses if you wear them, and a list of any medication you take. Bring your D4 form if you already have one.
Can I pay at the van?
Yes. It's £54.99 on the day. Booking and paying online is £49.99, so you save £5.
What if I don't pass?
The doctor records what they find on the D4 form. The licence decision is DVLA's, not the doctor's or ours. If anything needs checking again, your retest is free.
Who does the D4 medical?
A GMC-registered doctor. Our clinics are vans parked at transport sites and industrial estates, so there's no GP appointment to wait for.
Can I get the DVLA D4 form online?
You don't need to print one. Bring the D4 form DVLA sent you with your application pack, or use one of the spares we keep at the van.
Which licences need the D4?
C, C1, CE, D and D1: lorries, buses, coaches and minibuses. The same D4 form covers all of them, for a first application or a renewal.
